You can get started with Your Ticket Dollars by completing the inquiry form provided below. Once we receive your inquiry, we will call you to conduct career counseling to assist in developing the right Individual Work Plan (IWP) for you.
After we have discussed our services in more detail, answered your questions and determined that you would benefit from our services (and you wish to proceed with assigning your Ticket) Your Ticket Dollars will send you the following:

Individual Work Plan to review, sign and return to our office

A Funding Schedule* that shows the Career Supports Payments that you would qualify for and when you can request a Support Payment.

In order to confirm the type of Social Security Benefits you are receiving you should request a Benefits Planning Query (BPQY) and return it with your IWP. To obtain this form, please contact the Social Security Administration (SSA) office at 800-772-1213. SSA will send the BPQY directly to you. You may open their link here for more information:

Once we receive your signed Individual Work Plan, we will call you to discuss any other questions you may have and with your permission we will send your IWP to MAXIMUS for processing.
For Beneficiaries who are receiving SSDI benefits: Career Support Payments do not reduce SSDI payments.
For Beneficiaries who are receiving SSI (Supplemental Security Income) benefits: SSA may reduce SSI benefits for Unearned Income unless offset by acceptable work-related expenses.
SSA has work incentives outreach efforts to help beneficiaries plan for how income affects them. Work incentives specialists within SSA are available to provide assistance to help Beneficiaries anticipate and plan for the effect of work and earnings and other income on their SSI and/or SSDI benefits.
Several work incentives that could reduce your Income would be:
- The cost of impairment–related work expenses for items or services that a disabled person needs in order to work.
- Grants, scholarships, fellowships or gifts used for tuition and educational expenses
- The PASS PLAN: With a SSA approved Pass Plan, money you spend on your plan will not be counted when SSA determines SSI offset and eligibility.
Before returning to work or as soon as possible, you should consult Social Security or a Work Specialist at MAXIMUS to obtain information and assistance in making important decisions on how work may affect your benefits and how you may protect your benefits while you test your ability to become self-supporting. They can also explain how you can continue your Medicare or Medicaid coverage even when you earn your way off disability benefits.
